Stress, Complications, and Support Needs in German Neurosurgery: A Cross-Sectional Analysis

Abstract

neurosurgeons from 57 countries participated.Results: Departmental organization during parental leave was frequently limited; 90% reported absence of a structured coverage plan, often resulting in increased workload redistribution.Fertility concerns were notable, with 10.5% undergoing fertility treatment and reporting challenges attending medical appointments and concerns regarding professional perception.Among female respondents, 16.8% experienced pregnancy or postpartum complications and 13.4% reported miscarriage, with inconsistent access to protected recovery time.Barriers to breastfeeding, difficulty avoiding occupational risk exposure, and limited childcare infrastructure were commonly described.Parenthood was frequently perceived as negatively affecting training progression and career trajectory, including extension of training and reduced competitiveness, with recurrent reports of gender-based bias.Conclusions: Across diverse healthcare systems, pregnancy and parenthood in neurosurgery remain associated with structural limitations and perceived professional disadvantage.Standardized parental leave policies, equitable workload redistribution, protected recovery after pregnancy complications or loss, occupational risk mitigation, and institutional childcare and nursing support are important steps toward promoting equity and sustaining the neurosurgical workforce.
