Abstract

In geriatric dentistry, interforaminal implant placement of dental implants is standard to establish good retention of a mandibular prosthesis. The anatomy of the mental foramina as well as the inferior alveolar nerve and possibly its loop must be taken into account in order to be able to place 4 implants interforaminally.The aim of this study was therefore to establish a safety distance starting from the mental foramen in order to achieve a functional prosthesis treatment with minimal complications.For this purpose, 3D models were created from mandibular data sets (N=384) obtained from CT images in order to determine and measure the anterior loop and the incisive canal.The age distribution was 19-95 years (mean: 60 years), with a gender distribution of 1:2 (women:men). An anterior loop was detected in 90% of all patients examined. The mean loop length in women (left/right: 1.56/1.67 mm) was on average 18.75% and 17.73% significantly smaller compared to men (left/right: 1.92/2.03 mm). It was not only the prevalence and length of the anterior loop of the inferior alveolar nerve that diverged. The possible factors that can be used as a clinical categorisation aid, such as gender or side of the mandible, also do not provide any valid, useful information. Therefore, a standardised specification of a safety distance from a meaningful functional point of view is only possible to a limited extent.The assumption of previous studies that meaningful planning of such elective procedures for prosthetic treatment should only be carried out with prior 3D imaging in order to achieve the best possible result for the individual patient was confirmed. The use of a safety distance of 4 mm, as established in this study, can serve as a valuable guideline in the planning of an implant restoration in order to explore general feasibility, taking into account the individual patient requirements.
