Colorectal Cancer

Abstract

Colorectal cancer is the third most common cancer in terms of incidence and its clinical presentation, although evocative, may be indolent, therefore highlighting the necessity for population-based screening. Survival outcome of colorectal cancer is closely associated with cancer stage at initial diagnosis. Although challenged recently by alternative treatments for rectal cancer, surgical resection remains the primary treatment for colorectal cancer, and can be performed using different surgical approaches. Recent years have seen the development of robotic surgery and of transanal surgery for low rectal cancer, and of complete mesocolic excision for right-sided colon cancer, which bring more options to the surgeon's armamentarium. Moreover, neo-adjuvant treatment for rectal cancer may downstage the tumor or even allows complete clinical response with, in selected cases, possibility of entering a watch-and-wait program. On this aspect, immmunotherapy has definitely a role to play in patients with deficient mismatch repair system. Depending on the definitive pathological stage, different types of adjuvant therapies may bring some additional benefit in terms of survival.
